Use of Content

I understand that:

> My name may be used alongside the content unless I request otherwise.

> Content may be stored securely and used for as long as Christchurch Health Centre deems relevant.

> I will not receive additional compensation for the use of my image, voice or story.

Rights & Withdrawal

> I can withdraw my consent at any time by writing to Christchurch Health Centre

> Any materials already published before my withdrawal may continue to be used
