Christmas Availability Form

We kindly ask to staff to provide their usual availability over either Christmas OR New Year (or both should you wish!).
Once we receive all forms back we will confirm rotas with staff.
Any dates over the Christmas period that are not listed below we expect staff to work their usual rota.
